Resources for Your Rural Maternal Health Network

Tools and guidance to help build networks, implement innovative strategies, and use data to improve maternal health outcomes in rural communities

Improving maternal health in rural areas often requires close collaboration among clinics, labor and delivery hospitals, and social service programs. To support this, the Health Resources and Services Administration (HRSA) Federal Office of Rural Health Policy (FORHP) funds rural maternal health networks that expand access to care and improve outcomes for mothers and infants.

As of September 2025, HRSA has funded a total of 7 cohorts and 22 networks through the Rural MOMS and Delta MCC programs.

The resources and tools provided here were developed using data reported by these networks through the RMHD cooperative agreement. They can help you build and strengthen a network, implement innovative strategies across the full continuum of care, and use data to highlight and communicate your program's impact.
