Resources for Your Rural Maternal Health Network
Tools and guidance to help build networks, implement innovative strategies, and use data to improve maternal health outcomes in rural communities
Improving maternal health in rural areas often requires close collaboration among clinics, labor and delivery hospitals, and social service programs. To support this, the Federal Office of Rural Health Policy (FORHP) funds rural maternal health networks that expand access to care and improve outcomes for mothers and infants.
- Rural Maternity Obstetrics and Management Strategies (Rural MOMS) Program: Launched in 2019, this program funds networks to coordinate the continuum of care, leveraging telehealth and specialty care and improving financial sustainability of maternal health services.
- Delta Region Maternal Care Coordination (Delta MCC) Program: This 2024 program expands and improves health care services before, during, and after pregnancy for mothers who live in the Delta Region.
As of August 2025, HRSA has funded a total of 6 cohorts and 19 networks through the Rural MOMS and Delta MCC programs.
The resources and tools provided here were developed using data reported by these networks through the RMHD cooperative agreement. They can help you build and strengthen a network, implement innovative strategies across the full continuum of care, and use data to highlight and communicate your program's impact.
